Discharge Process

A plan for discharge should begin at the time a patient and family chooses a rehabilitation program by having a destination in mind and a commitment of working with the rehab team to achieve it.

An estimated length of stay will be determined on admission, goals established and a plan of care begun.

Patients/families will be involved in plan of care towards discharge discussion with Rehab physicians and designated staff. Patients are ready for discharge from the rehab program when one of the following situations occurs:

  • Patient has completed rehab goals and treatment potential has been reached.
  • Patient is medically unstable or a decline in status has occurred.
  • Patient's rehab functional status has reached a plateau and treatment potential has been reached. Patient elects to leave the rehabilitation unit against medical advice.
  • Patient is unable to participate due to extreme behavioral needs and is referred to special programming.
  • Patient is no longer requiring 3 hours of intense rehab therapy.

Before a patient is discharged medications and home instructions are reviewed and follow up appointments made according to your wishes and equipment required for home obtained.

Follow-up Care

All patients require some form of follow up care to assure they are making progress and have their medical needs met. This care may occur with a rehab physician, therapy provider, specialty clinic or outpatient program. We offer many outpatient services to your needs but if you choose not to have services in this area a staff member will help you locate appropriate programming near your home community.
