Breast/Ovarian and Colorectal Cancer Risk Assessment Programs
What the Program Entails
Because there are so many issues to consider in the process of risk assessment, the program begins with an initial educational session in either a group setting or as an individual session. For more information or to schedule a session, call (701) 234-2448.
If you would like to further your personal assessment you will be given the opportunity to schedule an individual informational session during which you will be asked to provide personal health information and fill out forms about your family history. This may involve talking with family members about their health history or getting information from medical records about their history, beforehand. It may also involve obtaining information from medical records of family members who have been diagnosed with cancer. Your individual risk assessment accuracy will depend on the accuracy of your family history.
After the Risk Assessment Team has reviewed your personal and family history, you will be asked to schedule a follow-up session during which the results and recommendations of your personal risk assessment will be shared with you. You will receive a copy of these recommendations for you to share with your primary care doctor. A summary of the risk assessment analysis will become a part of your medical record at MeritCare. This will be a reference for your doctor in your follow-up care. This information is kept confidential along with other information in your medical record. Your medical record is only released upon your written authorization or as permitted by law.
